I, the above person, hereby apply to become a member of PHF. I have read the Mission Statement of the Foundation and accept them. I am ready to dedicate some of my time and energy to the activities of the committee on a volunteer basis and would endeavor to expand these activities in my place of residence. The governing body of the Foundation is entitled to nullify my membership if it considers my activities against the declared goals of the Foundation.
